The 2014 Health and Lifestyles Survey. Questionnaire

The Health and Lifestyles Survey (HLS) is a biennial monitor of the health behaviour and attitudes of New Zealand adults aged 15 years and over, and parents and caregivers of 5 to 16-year-olds, first carried out in 2008. The HLS is managed by HPA and collects information relating to HPA’s programme areas of alcohol, tobacco control, skin cancer prevention, minimising gambling harm, nutrition, physical activity, immunisation and mental health. The data will be used to assess changes and developments in New Zealanders’ lifestyles as these relate to public health outcomes. The HPA uses these analyses to target at-risk groups more effectively and to efficiently promote healthy lifestyles

The questionnaire was developed by the Research and Evaluation Unit. Data collection for the HLS was contracted to CBG Health Research Limited.
