Aged-residential care gastro outbreak notification form — Northern
Submit this form to notify National Public Health Service of a gastroenteritis outbreak or suspected outbreak at an Auckland or Northland aged-residential care facility.
When to use this form
You should notify public health if one or both of the following apply.
- There are 2 or more cases of acute gastroenteritis in a 48-hour period in your facility.
- 2 or more cases of acute gastroenteritis are linked to a common source or event, such as a specific meal or celebration event, in your facility.
Public health will aim to respond to you within 1 working day.
